9. Occlusal Rehabilitation without Anterior Re-prosthesis

Case Summary

The patient had worn removable dentures for several decades without proper
restoration of the vertical dimension.
Progressive loss of occlusal height resulted in chronic mandibular retrusion,
temporomandibular joint symptoms, and insufficient prosthetic clearance
for posterior implant rehabilitation.

Despite functional impairment, the patient wished to avoid anterior re-prosthetic
treatment due to financial considerations.

Treatment Objectives

• Restoration of posterior occlusal height through bilateral prosthetic elevation

• Anterior–inferior repositioning of the mandible to improve jaw stability

• Relief of temporomandibular joint symptoms through occlusal correction

• Creation of adequate prosthetic space for posterior implant restorations

• Functional rehabilitation prioritised over anterior aesthetic refinement

INTRA ORAL - AFTER

Treatment Approach

Posterior occlusal vertical dimension was selectively increased to guide
mandibular repositioning and re-establish functional occlusion.

Although anterior aesthetic re-prosthesis was considered,
treatment was intentionally limited to functional rehabilitation
in accordance with the patient’s preferences and long-term sustainability.
